A Live Animal Consent Form is required for all students bringing a live animal onto campus for academic or class-related activities. This form, along with current vaccination records, must be submitted to the Laboratory Manager at least three days in advance to ensure the safety and welfare of everyone involved and to comply with university regulations.
In rare cases, the Laboratory Manager may grant an exception to the three-day rule at their discretion.

Excused absence requests are only necessary when a student will be missing a required lab, recitation, or exam. In accordance with the DVM attendance policy, you do not need to request an excused absence for simply missing a lecture, unless in-person attendance is specifically required by the instructor (e.g., guest lecturers, case days, TBL, Diagnostic Challenge, etc). Excused absences must fall into the category of legitimate reasons, per CSU's attendance policy (examples listed on the form). If the reason does not fall into that category, the absence will not be excused and the form is unnecessary.
Students must take the initiative to submit a completed excused absence request for approval. In the absence of extenuating circumstances, examinations must be completed within 48 hours of the original examination date and time.
